2024 Vendor and Sponsor Form

The Milwaukee Consortium for Hmong Health, Inc. (MCHH) is grateful for your interest in signing up as a Sponsor or Vendor at our 2024 Fall Health Fair on September 14, 8 a.m. to 12 p.m. at our office location at 1802 W Walnut Street in Milwaukee. **Set up begins at 7:30 a.m.**

Events like our Fall Community Health Fair and our spring Health Conference are our main ways to invite people to accept preventative care through health screenings. As always, we thank you for your partnership and your past support!

Event Sponsor $1000

Lunch Sponsor $500

Includes: Logo on all promotional material, postings on Facebook and LinkedIn acknowledging your sponsorship, thank you poster near lunch area, fee waived for booth set up. MCHH to provide 1 vendor table and 2 chairs for event sponsorship.

Bounce House Sponsor $250

Includes: Logo on all promotional material, postings on Facebook and LinkedIn acknowledging your sponsorship, thank you poster near Bounce House entry, fee waived for booth set up. MCHH to provide 1 vendor table and 2 chairs for event sponsorship.

Face Paint Sponsor $250

Includes: Logo on all promotional material, postings on Facebook and LinkedIn acknowledging your sponsorship, thank you poster near Face Painting area, fee waived for booth set up. MCHH to provide 1 vendor table and 2 chairs for event sponsorship.

Vendor $100

Includes: Name printed on promotional flyers; postings on Facebook and LinkedIn acknowledging your support. MCHH to provide 1 table and 2 chairs.

Basic Vendor *Fee waived
